The appointment request form
When you send an appointment request, we receive the name, phone number, email address and preferences you enter, plus your optional note. The request is emailed to our office so we can contact you to schedule a visit. We use it only for that purpose and to answer your questions.
Please don't include medical details in the form. Email isn't designed for sensitive health information — we'll go over your health history with you privately at your visit.
Patient forms
Our patient forms are PDF files you fill in on your own device. Nothing you type into them is sent to us or stored by this website. Please print them or bring them on your phone to your visit, and don't email completed forms.
Visitor counts
To understand how many people visit, our own server counts page views. We don't use cookies, advertising trackers or third-party analytics services, and we don't build profiles of visitors. Counts are kept as daily totals only.
Like all websites, our server briefly keeps standard technical logs (such as IP address, browser type and the page requested) to keep the site secure and working.
Sharing
We don't sell, rent or trade your information. We share it only with service providers who help us run the website and deliver email, or when required by law.
